"Falls and Fall Prevention"

Objectives
- Be able to categorize changes in mobility based upon targeted assessment and communicate possible interventions by engaging patients in the discussion.
- Be able to systematically perform a medication review in cases identified as a high falls risk or in a post-fall assessment.
- Perform a post-fall assessment quickly, efficiently and boost coding (E/M and HCC) in routine follow up visits.
- Evaluate the importance of using control charts to set thresholds and monitor falls incidences.
